How to Enroll for Diabetes Education
Physican or health care provider referral required. Print a referral form to take to your health care provider.
Medicare and most insurances cover up to 80% of the cost after deductibles are met if you are newly diagnosed. Medicare allows 10 hours initial training within 12 months of diagnosis, and 2 hours a year follow up training on individual and group basis.
When checking with your insurance plan ask if your plan covers, diabetes self-management training (DSMT).
Class Details
Diabetes Self-Management Classes cover areas specified in the National Standards for Diabetes Self-Management Education. Classes are taught by a diabetes educator and registered dietitians.
