Volunteer Applications

All applicants for volunteer positions need to download the Volunteer Application Disclosure and Clinical Student Pre-check Authorization Forms. These forms need to be completed and must accompany the appropriate application below.

Adult Volunteer Application

Junior Volunteer Application. Along with your application please include a letter of recommendation from a teacher, guidance counselor, and a (non-family) adult. In addition please include a copy of your immunization record and a signed copy of the responsibilities required for the Junior Volunteer Program as well as the Parental Consent Form.

College Student Volunteer Application. With your application please attach a letter of recommendation from one of your current professors.
